What is it about?

  • The Vitale card is a smart card containing administrative information like Social Security Number useful for medical professionals.

  • It is issued to any beneficiary of Health Insurance, from the age of 16, or from 12 years on request by the parent to whom the child is attached, from their Ameli account.

  • It confirms your social security registration and your rights to Health Insurance.

  • It is valid everywhere in France.

  • The healthcare professional you consult (doctor, dentist, etc.) uses it to establish an electronic treatment sheet (which replaces the paper).

  • The information is sent directly to your health insurance organization.

  • Thus, you are reimbursed within 7 days without having to take any steps.


Who is concerned?

  • The Vitale card is allocated to you as a beneficiary of health insurance from the age of 16.


What is the procedure to receive the card?

  • You do not need to do anything; the card is sent to you automatically by your health insurance organization after registration.

  • However, we can also apply for the first card online via assure.ameli.fr

  • It's simple, practical, free and you will receive your card faster.

  • When the card is sent to you, you will also receive a paper copy of the information stored in it.

  • In the event of an error or inaccuracy on the paper copy, you must report it to your health insurance organization.


How much does it cost?

  • It is provided free of charge by Assurance Maladie (Health Insurance). So, If someone asks you for money to issue you a card, it is a scam.


How to use card?

  • Present your Carte Vitale to any healthcare professional. He inserts it into his reader and transmits an electronic care sheet to your CPAM. This is called remote transmission.

  • If the healthcare professional does not have the necessary equipment, he establishes a paper treatment form. You must complete it and send this form by post to your primary health insurance provider to be reimbursed.

  • If you have forgotten your Vitale card, the healthcare professional can either ask you to sign a paper slip which he transmits to your primary health insurance fund (frequent case in pharmacies) or or establish a paper treatment form as mentioned earlier.


Should it be updated regularly?

When?

You must update your Vitale card

  • Once a year.

  • Every change of your personal or professional situation.

  • Change of address.


How?

To update the Carte Vitale, terminals are available in the following locations:

  • Health insurance organizations (CPAM).

  • Pharmacies.

If you cannot travel, contact your health insurance organization by phone or mail.


What should I do in case of loss or theft or malfunction of the card?

You can apply for a new card by using one of the following methods:

  • Make an Online request - Caisse nationale d'assurance maladie (Cnam) ameli.fr .

  • Visiting the CPAM office in your city.
